Brief introduction of the department: The psychogeriatric program is geared towards clients who are 55 years old and over, including those who suffer from mild cognitive impairment. The objective of the program is to maintain clients in their home environment as long as possible while continuously assessing the potential risks. Our nurse offers psychoeducation and psychopharmacology on various mental illnesses and on illnesses related to aging. We strive to prevent hospitalization when possible. If a client must be hospitalized, we facilitate the rehabilitation and reintegration in his or her natural environment.
This program is offered at the Centre Royal-Comtois Center.
List of services:• Screening for cognitive impairment and dementia.
• Continuous assessment of mental health symptoms, comprehension of the illness and response to treatment.
• Education on diagnosis, medication (daily management, comprehension, compliance, secondary effects, etc.) and on alternative treatments.
• Education on skills for healthy living.
• Facilitate acceptance and transfer to a residential style living.
• Post-hospitalization follow up
• Collaboration and common treatment planning with the treating psychiatrist and family doctor, the pharmacist and other partners in care.
• Active collaboration with the family members and other significant others (support, teaching, collaboration).
